San Gabriel Valley Music Theatre Inc’s COVID-19 Waiver
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19. The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. San Gabriel Valley Music Theatre Inc. (SGVMT) has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you or your child(ren) will not become infected with COVID-19.
By registering, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that your child(ren) and you may be exposed to or infected by COVID-19 by attending SGVMT and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 at SGVMT and on the grounds of the facilities used by SGVMT may result from the actions, omissions, or negligence of yourself, your child, and others, including, but not limited to SGVMT’s Board of Directors, volunteers, vendors, sponsors, partners, and program participants and their families.
You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to your child(ren) or yourself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you or your child(ren) may experience or incur in connection with your child(ren)’s attendance or participation in SGVMT programs.
On your behalf, and on behalf of your children, you hereby release, covenant not to sue, discharge, and hold harmless SGVMT, its Board of Directors, volunteers, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any Claims based on the actions, omissions, or negligence ofSGVMT, its Board of Directors, volunteers, and representatives,whether a COVID-19 infection occurs before, during, or after participation in any SGVMT programs or events.
PARENT/STUDENT consent to have student participate in The Broadway Locker Summer Musical Theatre Program at the San Gabriel Mission Playhouse and agree to hold the San Gabriel Valley Music Theatre, Inc., its agents, contractors, or employees harmless from any act of ordinary negligence, arising from any accident, injury, or damage whatsoever, however caused, to any person or persons, or the property of any person or persons, or corporations occurring during all times the student is participating in the program up to and including the time when the student is released into the responsible party’s care. San Gabriel Valley Music Theatre, Inc. reserves the right to withdraw any student from the program without refund due to misbehavior. Lastly, San Gabriel Valley Music Theatre, Inc. reserves the right to use photographs, video audio of the student taken during program hours for use in marketing collateral and promotion of San Gabriel Valley Music Theatre. I understand that these may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
Emergency Medical Authorization: I am the parent/guardian of the enrolled student. In the case that I am unable to be located during an emergency, I authorize a representative of San Gabriel Valley Music Theatre to act as an agent to consent to give medical care to the student listed on this online enrollment form.

allergies that your child might have that you believe we should be made aware of.
